1. Accomplish this order form until December 4, 2017 (MONDAY), 11:59AM

a.At Rm. 220, UP EEEI Bldg. II (Admin. Office), UP Diliman from November 21 until December 6. You may go there anytime between 8am-5pm.
b. Deposit it in the UP Electrical and Electronics Engineers Alumni Association, Inc. (UPEEEAAI) savings account until December 6, 2017 (WEDNESDAY), then send a copy of your deposit slip to: email@example.com

c. A reminder will be sent via email at the last day of reservation. Non-payment after 2 days will forfeit reservation of tickets. Kindly re-accomplish form if you still wish to avail of the tickets for the concert.
4. You may claim your ticket(s) until December 6, 2017 at Rm. 220, EEEI Bldg.II (Admin Office), from 8am to 5pm, OR you may also claim the tickets at the concert day itself, at the ticket booth (infront of Meralco Innovation Hall). Bring your receipt or your deposit slip as proof of payment.
